Your comprehensive Core Evaluation is comprised of four Evaluation Visits, each with a specific purpose, to help identify the root causes of your problems. This process concludes with Visit 5, design of your Initial Treatment Program.
Each Visit involves required assessment tools and education about the purpose of the tools. Please plan for approximately one hour of time at home (or at the office in advance of your visit) to accurately complete the surveys and to watch the associated videos.
