Zelis Provider Login

The login information is:

username: WYNDMOOR832

password: !AlkYmSt123!tb

Once logged in, go to Provider Settings. Click “Choose” under VRA and then the red box “Choose Direct ACH”. This will take you to the registration.

To enroll in VRA, you will also need the following information:

    • Your 9-digit Federal Taxpayer Identification Number (TIN) or Employer Identification Number (EIN)
    • Complete legal name of your institution, corporate entity, practice or individual provider
    • Your bank account routing transit number (RTN) or ABA Routing Number (bank account and routing number can be found the business check – see below for illustration)
    • If you wish have remittance data delivered via a Clearinghouse or FTPs site, have that information available.
    • Please be prepared to provide official name of the provider’s financial institution
