Your comprehensive Core Evaluation is comprised of four Evaluation Visits, each with a specific purpose, to help identify the root causes of your problems. Each Visit involves required assessment tools and education about the purpose of the tools. Please plan for one hour (at home or at the office in advance of your visit) to accurately complete the surveys and to watch the associated videos.
