History Form Copy
This history form will allow you to record your medical history, and to add to it as you move forward, and as you remember things from the past. In fact, our process is designed to connect your past with your present and future
When the pdf opens, its best to:
- CLICKÂ on the link below; a new browser tab will open with the document;
- DOWNLOAD the history.docx form to your desktop;
- RENAME the form with your name and date (i.e. “smith_jane_history_2022_12_15.docx”) and SAVE;
- SAVE the updated document to your desktop for your records and backup
- EMAIL document to staff@cohlife.org by dragging from your desktop into a new email and SEND
